Creatine Is Not Just for Gym-Goers: The Brain and Longevity Evidence

Creatine is one of the most researched supplements in existence — thousands of studies, decades of safety data, consistent efficacy. Yet it remains largely siloed in the world of sports performance, while its emerging applications in cognitive aging, brain health, and longevity receive far less attention.

What Creatine Does in the Body

Creatine is a compound synthesized from amino acids (arginine, glycine, and methionine) primarily in the liver and kidneys. About 95% of the body's creatine is stored in skeletal muscle; the remaining 5% is distributed throughout the brain, heart, and other organs.

Creatine's primary function is maintaining the ATP-PCr (phosphocreatine) energy system. When cells need energy rapidly — during intense muscular effort, cognitive demand, or cellular stress — ATP is hydrolyzed to ADP. Phosphocreatine donates a phosphate group to ADP, regenerating ATP and maintaining cellular energy supply. Without adequate creatine, this buffer system becomes depleted.

The Brain Connection

The brain is one of the most energy-intensive organs in the body, consuming roughly 20% of total metabolic energy despite representing only 2% of body weight. It is also rich in creatine kinase, the enzyme that manages the ATP-PCr system.

The relevance for aging is significant: brain creatine levels decline with age, and low brain creatine has been associated with cognitive impairment, neurodegenerative disease, and depression [1].

Cognitive Performance: What the Trials Show

A 2022 meta-analysis in Nutrients pooled data from 15 randomized trials examining creatine supplementation and cognitive performance. The analysis found significant positive effects on memory — particularly short-term memory and working memory — with the largest effects seen in older adults and in situations of mental stress or sleep deprivation [2].

A randomized trial published in Proceedings of the Royal Society B found that creatine supplementation (5g/day for 6 weeks) significantly improved working memory and intelligence scores in vegetarians — a population with lower baseline creatine intake — compared to meat-eaters who responded less strongly, suggesting dietary creatine status is a key moderator of response [3].

Particularly striking is creatine's role under conditions of metabolic stress. A study published in Psychopharmacology found that creatine supplementation largely prevented the cognitive decline and mood deterioration typically seen after 24 hours of sleep deprivation [4].

Depression and Mental Health

An area of growing research interest is creatine's role in depression. Several lines of evidence converge: depressed individuals show lower brain creatine levels on MRI spectroscopy; antidepressants increase creatine kinase activity; and multiple small clinical trials have found creatine augmentation of antidepressants produces faster and greater response than antidepressants alone.

A randomized trial published in the American Journal of Psychiatry found that adding 3-5g/day of creatine to an SSRI in women with major depression produced significantly faster response and higher remission rates [5]. This effect requires replication in larger trials but is biologically plausible.

Muscle Preservation in Aging (Sarcopenia)

Age-related muscle loss — sarcopenia — accelerates after age 60 and is a major driver of frailty, falls, disability, and mortality in older adults. Creatine supplementation combined with resistance training has been shown in multiple meta-analyses to produce greater gains in muscle mass, strength, and functional performance in older adults than resistance training alone.

A meta-analysis of 22 randomized controlled trials in adults over 55 found that creatine plus resistance training increased lean body mass by an additional 1.37kg and upper body strength by 7.7% compared to training plus placebo [6].

Dosing for Longevity Applications

The traditional loading protocol (20g/day for 5-7 days) is unnecessary for longevity applications and increases GI side effects. A maintenance dose of 3-5g/day achieves full saturation within 3-4 weeks and is well-tolerated in long-term use.

Creatine monohydrate remains the best-studied and most cost-effective form. Despite marketing for creatine HCl, ethyl ester, and other variants, no form has demonstrated clinically superior efficacy to monohydrate at equivalent doses.
