Magnesium L-Threonate and Brain Health: The Evidence
Magnesium is involved in over 300 enzymatic reactions in the body, yet most adults are deficient. Standard magnesium supplements (citrate, glycinate, oxide) are adequate for correcting systemic deficiency, but they do something important poorly: they don't meaningfully raise magnesium levels in the brain.
Magnesium L-threonate (MgT), commercially known as Magtein, was specifically designed to address this. Developed by researchers at MIT, it's the only form of magnesium that has been shown in controlled studies to significantly increase magnesium concentrations in cerebrospinal fluid.
The Blood-Brain Barrier Problem
The brain tightly regulates what enters it. Standard magnesium supplements raise plasma magnesium levels, but the blood-brain barrier limits how much makes it into the central nervous system. This matters because magnesium plays critical roles in the brain: it regulates NMDA receptors (central to learning and memory), modulates neuroplasticity, supports the myelin sheath, and reduces neuroinflammation.
The MIT team, led by neuroscientist Guosong Liu, screened magnesium compounds for their ability to penetrate the blood-brain barrier. Magnesium L-threonate — magnesium bound to threonate, a metabolite of vitamin C — substantially outperformed other forms in animal models, raising brain magnesium levels by approximately 15% compared to other forms [1].
Human Clinical Evidence
The pivotal human trial was published in 2022 in Nutrients. Researchers randomized 109 adults aged 18-65 with self-reported cognitive concerns to receive either Magtein or placebo for 30 days. The primary outcome was overall cognitive ability measured by a validated assessment tool.
Results showed that the MgT group demonstrated a clinically meaningful improvement in overall cognitive ability, with the most pronounced effects in executive function and attention. Notably, participants in the MgT group showed brain age scores (based on cognitive performance norms) that were approximately 9 years younger at the end of the trial [2].
An earlier randomized trial specifically targeting older adults with mild cognitive impairment found that 12 weeks of MgT supplementation improved cognitive scores significantly compared to placebo, with effects most notable in episodic memory and executive function [3].
Sleep Effects
Magnesium's relationship with sleep is well established — it modulates GABA receptors and reduces activity of the hypothalamic-pituitary-adrenal (HPA) axis, the body's primary stress response system. MgT's brain penetration makes it particularly effective for sleep-related applications.
A small randomized trial found that MgT improved both sleep quality and daytime function in older adults with insomnia, with effects comparable to commonly used sleep aids but without the cognitive side effects associated with prescription sedatives [4].
Dosing
Clinical trials have used 1.5-2g of elemental magnesium from MgT daily, typically divided into two doses (afternoon and before bed). Because the threonate component is the carrier rather than active ingredient, MgT products contain less elemental magnesium per capsule than other forms — you need more capsules to hit the therapeutic dose. Most products deliver the clinical dose in 3 capsules daily.
MgT should not be viewed as a complete magnesium supplement — if you have systemic deficiency, magnesium glycinate is more cost-effective for that purpose. MgT is specifically for neurological and cognitive applications.
Combination With Lion's Mane
Magnesium L-threonate and Lion's Mane mushroom are frequently combined in cognitive-support protocols. The rationale: MgT provides the substrate for synaptic transmission and plasticity, while Lion's Mane stimulates nerve growth factor (NGF) production, which promotes the formation and maintenance of new synaptic connections. The combination has not been tested directly in a clinical trial but has sound mechanistic support.
Safety and Tolerability
MgT has an excellent safety profile in published trials, with side effects comparable to placebo. The most common complaint is mild drowsiness, which most users experience as a benefit given the sleep effects. There are no known serious adverse events in clinical trial populations.
